Nurse Family Partnership Annual Fees Michigan Department of Health and Human Services (MDHHS) requests to enter negotiation with Nurse Family Partnership DBA Changent, as the vendor is the exclusive licensee of the Nurse Family Partnership (NFP) home visiting model that is implemented in Michigan. NFP model has been implemented in Michigan for many years and is a key part of the expanding home visiting services within the State to help address maternal and child health outcomes. Additionally, home visiting is a strategy listed in the MDHHS Infant Mortality Reduction State Plan. The vendor provides significant benefit to Michigan, including support and training that is provided to agencies implementing the model. The vendor also hosts a data system that is used to collect client data, which informs both our federal Maternal, Infant, and Early Childhood Home Visiting (MIECHV) grant reporting and State PA291 of 2012 reporting.
