Homeless to Housing Request for Proposals

The State of Connecticut, represented by the Department of Mental Health and Addiction Services (DMHAS), is soliciting proposals from providers. The purpose of this RFP is to develop programs to support people experiencing homelessness find and sustain stable, safe, permanent housing. Agencies will do this by implementing interdisciplinary service teams that address the individualized needs of each consumer. The service teams will follow the consumer from their unsheltered or sheltered situation through the stabilization of permanent housing. The program will follow a “services follows the person” model.
