Description
This grant supports organizations serving rural communities in Connecticut, using HRSA’s definition of “rural.” Remote Patient Monitoring (RPM), paired with AI‑enabled care management and assistive technologies, helps address geographic and workforce barriers by enabling real‑time health monitoring, earlier risk detection, and proactive interventions. Applicants are encouraged to propose innovative, technology‑enabled models for individuals with chronic conditions, developmental disabilities, and other high‑need populations. These solutions can promote independence, improve communication with providers and caregivers, and support health management in home and community settings. Integrating RPM and related tools into clinical workflows and EHRs enhances coordination, access, and continuity of care.
