On-Site Service Coordinator for Behavioral Health Housing Units

County Health, Behavioral Health and Recovery Services ( BHRS ) is looking for a qualified service provider to deliver onsite service coordination for eleven (11) behavioral health housing units at the Monarch at Redwood affordable h ousing development located at 2400 San Mateo Avenue, Redwood City, CA 94063 . BHRS behavioral health units target individuals living with serious mental illness (SMI) and/or substance use disorders (SUD) who are homeless or at risk of homelessness. The Monarch at Redwood development owned by Affirmed Housing California and managed by John Stewart Co. is a n 86 -unit affordable housing project with 11 units set aside for BHRS eligible tenants. Referrals for behavioral health units will be provided by BHRS . The Behavioral Health Services Act (BHSA) requires BHRS to maximize available Medi-Cal funding for eligible housing-related services. Tenants in the BHRS-designated units may be eligible for CalAIM Community Supports, including Housing Transition Navigation Services, Housing Deposits, Housing Tenancy and Sustaining Services, and Transitional Rent, as available through their Medi-Cal managed care plan. The selected provider will ensure tenants access available Community Supports benefits and billing the managed care plans and BHRS for the appropriate services.
