Provider Agreement (PA) - Alaska Breast & Cervical Screening

The Department of Health, Division of Public Health, is entering into agreements under 7 AAC 81 with qualified providers for the State of Alaska's Breast & Cervical Screening Program for the purpose of providing breast and cervical cancer screening and Screening Services (referred to herein as clinical services) to age and income eligible women enrolled in the State of Alaska’s Alaska Breast & Cervical Program.
