Revitalize Wellness Clinic, LLC is a San Antonio, Texas provider of diagnostic laboratory and medical support services to government and commercial clients. Clinical oversight is provided by our contracted Medical Director, an emergency medicine physician with 40+ years of practice, prior department chairman, research facility founder, and medical device inventor. Core capabilities: specimen collection and waived laboratory testing under a CLIA Certificate of Waiver; RN and LVN staffing support; IV nutrient infusion and injection therapy; weight management and hormone optimization programs; and medical wellness consultation. Our RN-led team brings more than 20 years of critical care, ER, home health, and hospice leadership experience, including disaster medical staffing and federal emergency management response. We are HHSC-enrolled, SAM.gov registered (UEI KTRWJ2NQWTA5, CAGE 14V41), and available for federal, state, and local subcontracting and teaming opportunities.
