Capabilities

CHDC advances a tech-enabled community health workforce to care for and connect high disparities, rural and Medicaid populations to their healthcare, behavioral health, social service and/or public health infrastructure. This is done through community based workforce training, curriculum design/development, program design/development and a technology platform. The platform allows all members of the Care Team to communicate, coordinate care, establish a bio-psycho-social Care Plan, perform closed-loop social service referrals and collect data over several performance areas, including quality, utilization, social drivers and disparities which feeds into a central dashboard for cross-program management. As well, our care pathways, screening questions, data fields and metrics/KPIs are customizable to the needs of a particular client or organization.
