We assist teams with developing strategic plans; quality and performance improvement programs; quality plans; team-based care models; and care management/care coordination programs. In addition, we provide expertise in obtaining Patient Centered Medical Home recognition and accreditation as well as guidance to improve ambulatory healthcare management, physician recruitment and retention, and patient engagement.
