Capabilities

We provide community based health monitoring for adults with disabilities and seniors. We are staffed with Registered Nurses, Licensed Practical Nurses, Certified Nursing Aides, Physical Therapists and Occupational Therapists to provide rehabilitative care to reduce need for permanent nursing home placement, frequent hospitalizations, and bedfast individuals. We provide nutritious meals- breakfast, lunch, and snacks to promote wellness and health for persons with chronic medical conditions to reduce exacerbations of diabetes, heart disease, and renal disease. We provide services to adults age 18 and older Monday-Friday 7 a.m.- 6 p.m. to allow caregivers respite care.
