Our mission is to improve the lives of those suffering from diet-sensitive disease and food insecurity, to contribute to health equity, and to bend the health care cost curve. Our goal is to sell superior personalized, tech-enabled, food solutions to health care payers to address diet?related health disparities and improve health outcomes for vulnerable individuals across the United States (US). Our customers are rapidly growing government-sponsored health plan businesses including Medicare Advantage (MA), Medicaid Managed Care Organizations and risk-bearing providers. We provide home-delivered food solutions, including prepared meals, meal kits and/or grocery bundles, along with nutrition education and coaching to support dietary change. Food solutions are customized according to individual clinical needs, preferences and circumstances. In 2022, we will also be offering mixed grocery bundles and produce boxes, together with meal preparation guidance.
