We are the home intelligence system that healthcare navigators need to reduce hospital readmissions and manage complex chronic conditions in rural communities. We support telehealth by continuously assessing and quantifying whole person functionality in the home, and by scanning for 175 emerging risk indicator behaviors related to sleeping, eating, moving around the home, and using appliances. We use privacy-protecting passive sensors so the home resident has zero burden for collecting data. We do not require cameras, audio, smartphones, or wearables. Our system includes a cellular link for encrypted data transmission so the home does not need existing broadband or WiFi. We are a proactive care intelligence platform eliminating barriers to care for non-digital populations. Our business model is to partner with healthcare payer/providers as their care navigation tool for high risk patients. Our revenue model is subscription + equipment.
