DDTP Diabetes Surveillance Portland Area

The Portland Area provides Area-wide tracking of diabetes prevalence in AI/AN people in the Portland Area. The Area tracks the prevalence and incidence of complications from diabetes. All Portland Area SDPI grant sites are provided with Tribe-specific and aggregate summary reports, such as the Health Status Report, the annual Portland Area Aggregate Report (summarizing the Annual Diabetes Care and Outcomes Audit) and the Portland Area Tribal Aggregate Report. This data helps grantees direct clinical and primary prevention programming. The data surveillance team makes site visits to regions, based on the needs assessment survey results.
