Description

Special Instructions to Offerors: The Government intends to award a single purchase order resulting from this RFQ to the responsible vendor whose quotation represents the best value to the Government, considering price and non price factors. The Government reserves the right to make award to other than the lowest priced vendor if a higher priced quotation provides superior value to the Government. Evaluation factors: Factor 1 Technical Acceptability (Pass/Fail) The quotation will be evaluated to determine whether it demonstrates a clear ability to meet the requirements of the statement of work. Technical acceptability will be evaluated based on: (A) Schedule. The quoter shall submit a schedule based on a mock Notice to Proceed of 10/1/2026. The Government will evaluate whether the schedule demonstrates the ability to meet or exceed the required completion date(s) and reasonably minimize disruption. (B) Technical approach and understanding. The quoter shall provide a plan describing demolition (if required), subfloor preparation, moisture mitigation, installation of new flooring, protection of adjacent finishes, and cleanup. The Government will evaluate whether the approach reflects an understanding of the requirement and a reasonable likelihood of successful, high quality performance. (C) Quality assurance and safety plan. The quoter shall provide a plan describing inspections of work, adherence to manufacturer installation requirements, safety procedures (including OSHA compliance), and housekeeping during installation. The Government will evaluate whether the plan is adequate to ensure quality and safety. Technical acceptability is rated Pass or Fail. A quotation rated Fail under any of the above elements may be ineligible for award. Factor 2 Product Sample (Pass/Fail) For a quotation to be considered for award, the quoter shall provide: A minimum 3" x 3" sample of the actual flooring proposed, or a manufacturer color chart if an exact sample of the requested color is unavailable, and the manufacturer s written specifications for the proposed product. Physical samples must be dropped off prior to the offer due date at Building 108 the Whole Health Wellness Center. Mark all samples/ packaging with the SDVOSB firm that is submitting the offer and 36C25626Q1005 Attention Dr. Vivian Bassbee Whole Health Wellness Team Renovation Director. The Government will evaluate whether the proposed product appears to meet the required characteristics and color requirements. Quotations that do not provide the required sample and specifications may be rejected as non responsive to the RFQ. Factor 3 Past Performance The Government will evaluate the quoter s past performance to assess the likelihood of successful performance. The Government may consider information from the contracting officer s knowledge, CPARS, customer surveys, or any other reasonable source. The Government is not obligated to contact the quoter for additional past performance information. Factor 4 Price Price will be evaluated for reasonableness. The Government will compare total prices among quotations and determine whether the proposed price is fair and reasonable. The Government will then determine which quotation represents the best value to the Government, considering price and the non price factors listed above.
