Conducting, Tabulating and Reporting on Surveys

The Vermont Division of Vocational Rehabilitation (hereafter “DVR”) is a statewide direct service organization within the Department of Disabilities, Aging and Independent Living in the Agency of Human Services; DVR provides a wide range of employment-related services for people with disabilities. The Contractor will conduct, tabulate and report on two (2) separate surveys as follows: • Task 1 - Consumer Satisfaction Survey (CSS): Conduct, tabulate and report on one (1) survey to determine current consumer satisfaction with DVR services; • Task 2 - Monthly Consumer Surveys for Staff Performance Review (SPR): Conduct, tabulate and report on regular, ongoing consumer surveys to determine satisfaction with services from individual counselors as a part of DVR’s staff evaluation process.
