Department of Veterans Affairs Salem VA Medical Center 1970 Roanoke Blvd, Salem, VA 24153 Statement of Work (SOW) Project Title: Dock and Dock Plate Replacement – Buildings 14 & 15 Facility: Salem VA Medical Center (VAMC) Date: May 14, 2026 1. Scope of Work The contractor shall furnish all labor, materials, equipment, and supervision required to remove and replace existing dock plates and perform associated concrete and structural work at Buildings 14 and 15, Salem VAMC. All work shall comply with VA standards, local building codes, and OSHA safety requirements. 2. Description of Work Building 14 Remove existing dock plate. Sawcut and remove approximately 14 ft × 50 ft of asphalt. Remove approximately 50 ft of existing curb. Excavate for new footing at existing dock wall to extend dock area. Form and pour new 4 ft × 1 ft × 14 ft retaining wall with 4×4 steel angle on edge. Form and pour new 6 in × 18 in × 50 ft curb. Form and pour new concrete drive (14 ft × 50 ft × 6 in), sloped to allow trucks to back up level with dock. Extend existing handrail by 2 ft. Install new 4 in × 10 in × 14 in rubber dock bumpers. Building 15 Remove existing pit leveler and associated equipment. Infill existing pit with #57 gravel. Sawcut back of pit at cantilever. Pour new 4 in concrete slab pinned to existing foundation. Install new 4 in × 4 in pit leveler frame and pour concrete perimeter. Install new 6 ft × 8 ft hydraulic pit leveler and 4 in × 10 in × 14 in dock bumpers. 3. Performance Standards All work shall: Meet VA design and construction standards. Comply with Applicable Sections of the VA Electrical, Structural, and Safety Guidelines. Be executed following OSHA safety practices. Be completed per approved construction drawings and specifications. Ensure new concrete and metal installations are level, aligned, and finished to VA quality standards. 4. Schedule and Coordination Work schedule shall be coordinated with the VA Engineering Service prior to mobilization. Contractor shall provide a minimum 48-hour work notice for any activity affecting operations. Access to work areas shall be limited to authorized personnel only. 5. Safety and Environmental Requirements The contractor shall maintain a safe work environment at all times. All waste materials shall be removed and disposed of in accordance with EPA and local regulations. Contractor shall ensure dust control and minimal disruption to medical center operations. 6. Inspection and Acceptance All work will be inspected by the VA contracting officer’s representative (COR). Final acceptance will occur after successful inspection and demonstration of functionality of all installed components. Period of Performance is 120 days from issuance of Notice to Proceed (NTP).
