External Evaluation Services - RFP 16849

Horizon Behavioral Health (Horizon) is soliciting proposals from qualified independent evaluators to provide comprehensive evaluation services for its Substance Abuse and Mental Health Services Administration (SAMHSA) Community Programs for Outreach and Intervention with Youth and Young Adults at Clinical High Risk for Psychosis (CHR-P) grant. The selected evaluator will support implementation, performance measurement, continuous quality improvement (CQI), federal reporting, and assessment of process, implementation, fidelity, and outcomes throughout the anticipated four-year project period while maintaining sufficient independence to provide objective findings and recommendations.
