COMBINED SYNOPSIS/SOLICITATION Notice Type: Combined Synopsis/Solicitation Solicitation Number: 75H70626Q00184 Title: Great Plains Area Indian Health Service (GPA IHS) Hematology Analyzer Cost-Per-Reportable-Result (CPRR) IDIQ Contract 1. General Information This is a 100% Total Small Business set-aside combined synopsis/solicitation for commercial products and commercial services prepared in accordance with FAR 12.603 and FAR Part 13, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotations are being requested and a separate written solicitation will not be issued. The Indian Health Service (IHS), Great Plains Area Office, intends to award a single Firm-Fixed-Price, Indefinite-Delivery/Indefinite-Quantity (IDIQ) contract utilizing a Cost-Per-Reportable-Result (CPRR) pricing model for hematology analyzers and related supplies and services. The anticipated contract consists of one (1) base year and four (4) one-year option periods. The estimated contract ceiling is less than $2M and minimum guarantee is $1,000.00. 2. Description The Contractor shall furnish all labor, equipment, reagents, consumables, installation, validation, training, maintenance, warranty, software, interface support, and related services necessary to provide hematology analyzers for Great Plains Area Indian Health Service facilities located throughout South Dakota and North Dakota. Scope of Work The Contractor shall provide, at a minimum: New hematology analyzers Required peripherals Initial reagents, controls, and calibrators Delivery and installation Calibration and validation LIS interface support On-site training Warranty Preventive maintenance Repair services Software updates Technical support Complete requirements are contained in the attached Performance Work Statement (PWS). 4. Salient Characteristics Equipment shall include, at a minimum: Automated CBC with 6-part differential Immature granulocyte reporting Open and closed tube sampling Pediatric tube capability Barcode reader STAT capability Automated reagent management FDA-cleared body fluid capability (where applicable) HL7/LIS compatibility Continuous calibration capability Small tabletop footprint Additional system requirements by facility are contained in Appendix B of the PWS. 5. Contract Type Firm-Fixed-Price IDIQ, pricing shall be based upon a firm-fixed CPRR model. The CPRR price shall include all: Equipment Reagents Controls Consumables Maintenance Repairs Training Software Interface support No additional charges will be permitted. 6. Period of Performance One (1) Base Year plus four (4) one-year option periods. 7. Delivery Requirements The Contractor shall: Deliver equipment upon award/task order. Complete installation within ten (10) calendar days. Complete validation within thirty (30) days. Conduct onsite user training during installation. Provide documentation and warranty information with installation. 8. Warranty and Maintenance Minimum requirements include: Twelve (12) month full warranty Parts Labor Travel Preventive maintenance Software upgrades 24/7 technical support Onsite service response: Hospitals: within 48 hours Clinics: next business day 9. Evaluation Award will be given to the responsible offeror whose quotation represents the best value to the Government. Evaluation factors include: Technical Acceptability (Technical acceptability includes compliance with the PWS and all salient characteristics.) Price (Price will be evaluated for fairness and reasonableness.) Contractor Responsibility INTENT TO AWARD WITHOUT DISCUSSIONS The Government intends to evaluate proposals and award a contract without discussion with Offerors. Therefore, the Offeror's initial proposal should contain the Offeror's best terms from a price and technical standpoint. The Government reserves the right to conduct discussions if the Contracting Officer later determines them to be necessary. 11.
