Family Impact Initiative

The overarching goal of the program is to support the development of community based and family-focused systems of care for families of CSHCN. The core components include assembling an effective coalition of stakeholders, assessing community systems and identifying areas of improvement, developing and implementing strategies, building capacity to undergo systems change, improving the community service delivery system, and ensuring CSHCN get the support and resources needed to thrive.
