Description
Washington County Public Health (WCPH) is seeking proposals from eligible organizations to expand access to mobile healthcare services and social supports in rural Washington County communities. This project supports implementation of the Washington County Community Health Improvement Plan (CHIP). Funding for this opportunity is provided through the Oregon Health Authority (OHA) Rural Health Transformation Program (RHTP). OHA receives this funding from a federal initiative administered by the Centers for Medicare & Medicaid Services (CMS). Organizations funded through this Request for Proposal (RFP) will work in partnership with WCPH to implement approved activities that support the goals of this project. This funding opportunity supports community-based approaches that: Expand existing mobile healthcare services in rural communities Improve connections to social supports and community resources Organizations may apply under one or both priority areas described in this RFP.
