Description

The Mississippi State Department of Health, through the Mississippi Delta Health Collaborative (MDHC), is seeking an in-state School of Pharmacy partner to provide implementation support for the MDHC Medication Therapy Management (MTM) Program. This partnership will support the expansion and sustainability of MTM services delivered by community pharmacists to improve hypertension and high cholesterol outcomes across the 18-county Mississippi Delta region. The selected partner will collaborate with MDHC to strengthen community–clinical linkages, support training and engagement of interprofessional healthcare teams, participate in the MTM Advisory Task Force and the Mississippi Alliance for Cardiovascular Health (MACH) Learning Collaborative, and assist with community outreach and screening activities in the Mississippi Delta. Additionally, the partner will contribute to workforce development by creating and supporting pharmacy student/resident learning experiences focused on rural health and population health strategies. The partner will also support dissemination of program activities and outcomes by developing and contributing to presentations, reports, and other communication products to share best practices, lessons learned, and program impact with stakeholders, partners, and broader public health and clinical audiences.
