NAICS (North American Industry Classification System) code 339113 Surgical Appliance and Supplies Manufacturing. 5. State if your firm is a large business, small business, service-disabled-veteran-owned small business, veteran-owned small business, HUB zone 8(a), women-owned small business or a small disadvantaged business. 6. State if your firm has the required items on Schedule (FSS, GSA, SEWP, other ). 7. Provide a letter from the manufacturer verifying your firm is an authorized distributor. 8. Provide the country of origin for each of the required items. REQUIRED ITEMS: The government is seeking information from capable sources that can provide a ceiling-mounted bariatric patient lift system meeting the following general requirements: Ceiling-mount patient lift/hoist system Minimum lifting capacity of approximately 800 lbs Rechargeable/battery-powered operation Handheld and/or remote controls Smooth, controlled lifting and lowering Emergency stop and emergency lowering safety features Quiet operation suitable for a clinical environment Ceiling track system Capable of supporting bariatric patient transfers System capable of modification or expansion, as applicable Maintenance/service notification or tracking capability Compliant with applicable patient-lift and medical equipment safety standards Includes all necessary equipment The Government s anticipated requirement is for a Guldmann GH3+ bariatric ceiling lift system or equal. Respondents offering an equal product should identify the manufacturer and model proposed and provide product literature demonstrating the product s capabilities.
