Brookville Lake Dam Road Crack Fill and Seal

This Request for Quote (RFQ) is for the winning contractor to furnish all supervision, personnel, equipment, supplies, and materials necessary to perform pavement crack filling and seal coating services on the Brookville Lake Dam Access Road. This project is located at 10064 Overlook Rd, Brookville, IN 47012. The contractors work and responsibility shall include all planning, programming, administration, management, supervision, communications and inspection necessary to assure that all work is conducted in accordance with the contract requirements and all applicable Federal, State and Local laws and regulations. All work shall be performed in strict compliance with the attached specifications and all other terms and conditions incorporated herein. A site visit is recommended. Fax, telephones, mailed, or emailed submittals will NOT be accepted. ***Amendment 0001*** The Price Breakout Schedule (PBS) has been revised to clarify which areas are included in the estimated 87,500 square feet of work to be performed. The attached PBS specifies that the workarea includes the dam road, turnaround, and parking lot off of the turnaround as shown in Exhibit D of the Statement of Work (SOW). The total size of the work to be performed remains unchanged from the original estimate of 87,500 square feet. The offeror due date is hereby extended to 17 September 2026 at 1 PM EDT. All offers must acknowledge all amendments to the solicitation to be a responsive offer. All offers must use the revised PBS included with this amendment.
