SS - CHANGE ORDER NOTICE - Rate Developer Services

The purpose of Change order is to request for additional time to allow Guidehouse to continue working on the implementation of the Rate Calculator. IDHS/DDD had contract with Guidehouse effective 09/01/2023 through 12/31/2023 to provide communications and technical assistance to both IDHS/DDD and our community based providers of Community Integrated Living Arrangement (CILA) services. IDHS/DDD want to extend the contract through 06/30/2024.
