CHANGE ORDER NOTICE: CO#1. This change order is needed to adjust the items quantities. Contract change order is required to increase the contract in the amount of $83,580.00 for a new contract total of $762,854.78 This change order is a 12.3% increase to the current contract amount. These circumstances were unforeseeable at the time and germane to the original contract. Change order is in accordance with 720 ILCS 5/33E-9. (ref. original contract - P-4692)
