Description
The Department of Health ("DOH") requests input and comments from current and prospective providers for year 4 the Centers for Disease Control and Prevention ("CDC") notice of funding opportunity ("NOFO") CDC-RFA-DP-23-0014: Racial and Ethnic Approaches to Community Health (“REACH”). This multi-component 5-year program aims to improve health, prevent chronic diseases, and reduce health disparities among racial and ethnic populations with the highest risk, or burden, of chronic disease, specifically among African American, Black, Hispanic and Latino, Asian American, Native Hawaiian/Other Pacific Islander, American Indian, and Alaska Native populations. The project will support evidence-based, culturally tailored strategies at the local level among Native Hawaiian and Other Pacific Islanders (“NHOPI”) through prevention efforts that focus on interventions and activities for nutrition and physical activity. Written comments in response to this RFI may be transmitted via email to Heidi Hansen Smith at heidi.hansen-smith@doh.hawaii.gov. The deadline for receipt of comments is 4:30 p.m. on Tuesday, August 4, 2026.
