Pilot Project: Integration of Community Health Workers (Chws) in HawaiʻI Doh Cdphnd Service Delivery in Rural Communities

The Hawaiʻi Department of Health Communicable Disease and Public Health Nursing Division is soliciting proposals under RFP No. HTH-PHN-1-26 to manage administrative and workforce infrastructure for 6.0 FTE Community Health Workers serving rural communities statewide. A single cost-reimbursement contract up to $658,000.00 is anticipated to be awarded for October 28, 2026, through July 31, 2027.
