The City of Tampa Human Resources & Talent Development department is intending to single source onsite case management independent of health insurance company. The prospective bidder must be a provider of onsite case management independent of a health insurance company to potentially include but not be limited to: Embedded On-Site Presence, Full-Service Benefits Navigation, Hospital & Discharge Coordination, Worksite Integration, Proactive Data-Driven Specialty-Focused Outreach, and Comprehensive Specialty Support Team including a nutritionist, clinical pharmacist, licensed social workers, and outreach coordinators providing whole-person care that addresses physical, behavioral, and social determinants of health.
