Description

Purpose: The grant purpose is to provide funding to communities across the state to plan and develop their own, cross-sector local age- and disability-friendly plans. Under the grant program, CDA will support local communities in planning and creating their own age and disability-friendly action plans. The local plans developed under this grant program aim to improve a community’s livability and address the current, emerging, and future needs of California’s older adults, people with disabilities, caregivers, and families through cross-sector collaboration and transformational systems change. The purpose is to provide funding to communities across the state to plan and develop their own, cross-sector local age- and disability-friendly plans that: 1) transform the infrastructure and coordination of services and supports for older adults, people with disabilities, caregivers, and families; 2) Elevate the strengths and voices of community members who are aging and/or living with a disability; 3) Expand culturally relevant and accessible services and opportunities to older adults, people with disabilities, caregivers, and families; 4) Address health and other inequities that can become cumulative with age; and 5) Combat racism, ageism, ableism, and biases in aging and disability systems of care. The grant program goals align with California’s Master Plan for Aging (MPA). Eligible Applicants: California-based agencies and organizations that represent and serve a defined community. Under the RFA, community is defined as a city, a group of cities, a county, a group of counties, or Tribal land(s). Applicants may submit proposals as a single organization or on behalf of a consortium of organizations. Applicant may be community-based non-profits and established coalitions that are classified as 501(c)(3) tax exempt under the IRS Code. Eligible Geographies: -Rural & frontier communities -Tribal communities -American Indian or Alaska Native; Black; Asian; Native Hawaiian; and Pacific Islander -persons/communities/populations -People with disabilities -People living with Alzheimer’s disease and other dementias. -LGBTQ+ persons/communities/populations -Veterans -Professional & family caregivers -Persons/communities/populations of lower incomes -Persons/communities/populations of immigrant and/or refugee status Matching Funding: n/a
