Description
SFSO, is seeking qualified Proposers for Discharge Planning Services, which provides a range of reentry services to a diverse population with the specific goal of reducing recidivism. Comprehensive jail discharge planning, diligent case management, and strong partnerships with community-based programs are critical to connecting vulnerable reentry populations to relevant, effective services. Incorporating housing support as a component of the spectrum of services can often be the determining factor between success and recidivism. *Do not submit proposals through the Supplier Portal.
