Background The Eastern Oklahoma Veterans Health Care System has a requirement to purchase two Peracetic Acid Monitoring Systems, one at the Jack C. Montgomery VAMC in Muskogee, OK and one at the Ernest Childers Outpatient Clinic in Tulsa, OK. This system will be used in the Endoscopy Clinic areas where Peracetic Acid needs to be monitored for safety. Description Purchase and Installation of two Peracetic Acid Monitoring Systems with the following characteristics: Real time monitoring by continuously measuring for hazardous levels of Peracetic Acid Vapors in the air Audible and visual alarm when hazardous levels of Peracetic Acid Vapors are detected in the air Fixed Display Monitor Touch screen Displays real time measurements Wall mounted on a steel swivel wall mount Installation Assess each area in Muskogee and Tulsa Determine optimal locations for monitoring systems that consist of monitors and sensors Ensure proper placement of monitoring systems Ensure full functionality of monitoring systems after installation Training Train users at both sites in Muskogee and Tulsa Training should include Proper use Meaning of Alarms and how to respond Maintenance of system and how to utilize maintenance program Computer and Monitor for Data Tracking Must be standalone unit Cannot connect to the Network or Internet Direct connection to Fixed Display Monitor via cable Maintenance Program Provide all maintenance and calibration of Peracetic Acid Sensors Base Year Option Year 1 Option Year 2 Option Year 3 Option Year 4 Delivery Warehouse: Jack C. Montgomery VAMC 1011 Honor Heights Drive Muskogee, OK 74401 Scope The contractor will have the entire system delivered to the Jack C. Montgomery VAMC. The contractor will provide installation in the Endoscopy Departments Scope Processing Rooms at the Jack C. Montgomery VAMC in Muskogee, OK and the Ernest Childers Outpatient Clinic in Tulsa, OK. Contractor delivery and installation of equipment shall take place between the hours of 0800 -1500. Vendor shall not be required, nor is authorized to perform any work on the following US Government holidays: New Year's Day, Martin Luther King Jr's Birthday, Washington's Birthday, Memorial Day, Juneteenth National Independence Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day POC Muskogee: Robert Neuzil - SPS Supervisor, Robert.Neuzil@va.gov Tulsa: Tina Martin - Sterile Processing Technician, Tina.Martin3@va.gov Christy Iven-Jorgensen - Acquisition Utilization Specialist, Christina.Iven@va.gov Contracting: Christopher.delauro@va.gov CONTRACTOR FURNISHED ITEMS AND RESPONSIBILITIES The Contractor shall furnish all supplies, equipment, facilities, personnel and services required to perform the service under this contract. The Contractor shall perform all work in accordance with this Statement of Work. NON-PERSONAL SERVICE STATEMENT The C&A requirements do not apply, a Security Accreditation Package is not required. Contractor employees performing services under this order will be controlled, directed and supervised at all times by management personnel of the contractor. Contractor employees will perform independent of and without the supervision of any Government official. Actions of contractor employees may not be interpreted or implemented in any manner that results in any contractor employee creating or modifying Federal policy, obligating the appropriated funds of the U.S. Government, overseeing the work of Federal employees, providing direct personal services to any Federal employee or otherwise violating the prohibitions set forth in Parts 7.5 and 37.1 of the Federal Acquisition Regulation (FAR). The Government will control access to the facility and perform the inspection and acceptance of the completed work. Contract Officer Representative (COR). The COR monitors all technical aspects of the contract and assists in contract administration. The COR is authorized to perform the following functions: assure that the Contractor performs the technical requirements of the contract: perform inspections necessary in connection with contract performance: maintain written and oral communications with the Contractor concerning technical aspects of the contract: issue written interpretations of technical requirements, including Government drawings, designs, specifications: monitor Contractor's performance and notifies both the Contracting Officer and Contractor of any deficiencies; coordinate availability of government furnished property, and provide site entry of Contractor personnel. The COR is not authorized to change any of the terms and conditions of the resulting order.
